He was the son of a violinist and a pianist, and his parents recognized his musical talent at a young age. Guarnieri began playing the piano when he was just four years old, and by the time he was a teenager, he had already won several local competitions. Guarnieri's musical education began at the Milan Conservatory, where he studied under the tutelage of some of the most respected teachers in Italy. He quickly distinguished himself as a gifted pianist, and his talent was recognized by the Italian government, which awarded him a scholarship to study in the United States. In 1947, Guarnieri moved to New York City to study at the Juilliard School of Music. He quickly made a name for himself in the American music scene, and he was soon performing in some of the most prestigious concert halls in the country. In 1950, he won the prestigious Naumburg International Piano Competition, which helped to launch his career as a soloist. Over the course of his career, Guarnieri performed with some of the world's most renowned orchestras, including the New York Philharmonic, the Boston Symphony Orchestra, and the London Symphony Orchestra. He was also a frequent performer at music festivals around the world, including the Salzburg Festival, the Edinburgh Festival, and the Tanglewood Music Festival. Guarnieri was known for his virtuosic performances of the works of classical composers such as Beethoven, Mozart, and Chopin. He was particularly renowned for his interpretations of the works of the Romantic composer Franz Liszt, and he was widely regarded as one of the greatest Liszt interpreters of his time. In addition to his performances as a soloist, Guarnieri was also a respected chamber musician. He frequently collaborated with other musicians, including the violinist Isaac Stern and the cellist Yo-Yo Ma. He was also a dedicated teacher, and he taught at several prestigious music schools, including the Juilliard School and the New England Conservatory of Music.
